To submit your sample, please fill out one billing address form, the sample information sheet for each sample and sign the sample dating agreement paper.
Mail in your samples to the address below with the above completed documents.
Your invoice will be sent to you shortly after we receive your samples.

Samples of soil and sediment must be submitted wrapped in aluminum foil, and contained in a sealed container. This is also suggested to submit any type of sample.
